We like to get paperwork out of the way before your first session, so we can spend more time getting to know you. Therefore, we ask that you complete the following steps:<\/p>\n

For clients 18 years of age and older<\/span>, please print and fill out the Client Information Form<\/a>.\u00a0\u00a0<\/p>\n

Also, please read our Office-Policies-Procedures-and-Information<\/a>, Telemental Health Informed Consent<\/a>, and Practices-Regarding-Your-Protected-Health-Information<\/a>.\u201d<\/p>\n

Print and sign the Consent to Treat form<\/a> (right-click and select “Save Link As” to download). Signing this form indicates that you have read, understood, and agreed to the Office Policies, Telemental Health, and Privacy Practices linked above.<\/p>\n

Please bring the completed Client Information and signed Consent to Treat forms to your first appointment, or in the case of telehealth, please email or mail the forms at least 3 days prior to your first appointment.<\/p>\n
